the spleen is the most commonly injured abdominal organ, occurring in approximately 25% of all patients with abdominal trauma evaluated by ct. splenic injury can sometimes present as tenderness on palpation of the left upper quadrant (50-60% incidence) or as hypotension (30% incidence). contrast enhanced ct has shown a 95% sensitivity for diagnosing splenic injury and for this reason has become the method of choice in evaluating patients with blunt trauma to the spleen.

这是包膜下出血吗，应为脾周积血（腹腔内）。包膜下出血应为脾外伤最轻的一种，肯定不会引起腹腔积血，如腹腔有积血，只能说明包膜己破。

这种情况不可能作增强扫描，时间就是生命；需要密切结合病史与临床，了解血压情况，有时在检查后亲自询问病人甚至按压腹部；如果还不能明确，需要与外科医生交流，再检查b超，必要时穿刺抽液证实，再危机时开腹探察。
